Birthday Announcement

Deadline for submission for the weekly Wednesday paper is 5 p.m. on Friday. A submission constitutes this form and one photo for adults celebrating 80 or more years, in five year increments (80,85,90,95,100,etc.), annually after 100. Birthday announcements for children under 10 will also be accepted. The name and phone number of a contact person must be included on this form.
